How do you typically decide how much to tip your server?

I always tip a standard percentage (e.g., 20%).
I adjust the tip based on the quality of service.
I tip a flat dollar amount regardless of the bill.
I rarely or never tip.
It depends on the establishment or state/country I’m in.
